FILING YOUR CLAIM
While we are an out of network provider, many insurances still cover our services in part. Self-filing is also referred to as pay-and-claim which is what some local physician and chiropractic offices offer as well. Though it is not the most common way to file it actually reduces overall costs to both the client and provider by heavily reducing total administrative workloads, i.e., we don’t have to raise prices to either hire someone to file all the insurance claims. This in turn results in a lower bill to your insurance, higher likelihood of reimbursement and coverage, and lower total healthcare costs.
As we know this is different for most we will help you with the process as much as we can. Use the resources below or we can help you in person.
How does it work?
Full payment is made at time of service by the client.
A superbill is generated which has the information required by the insurance (unless your insurance also requires visit notes) and this is given directly to the client.
The next step differs slightly depending on your individual plan but generally one must submit it to their insurance either by faxing, via an online portal, or mail.
Reimbursement is sent directly to the client or applied directly to your deductible.
We also accept HSA and FSA cards.

Sign in to your health plan account to find your submission form
Sign in to your health plan account and go to the “Claims & Accounts” tab, then select the “Submit a Claim” tab. There, you’ll be able to select the Medical Claims Submission form to download and print.

Submit your claim by mail
After you print and complete the Medical Claims Submission form, mail it with the claim details and receipts to the address on your health plan ID card.
